BELLWOOD ILL. U.S.A. 115V AC/DC/AMP. MAX."The top the lid say "CAMPBELL PROD. CO. BENSENVILLE, ILL. LIFT OVER BY THE OUTER EDGES," and the bottom side of the lide says "59-A".
