Each form measures 4" by 10"Up at the top where there is a place for the date it says 196__ so this is from the 1960'sTotal of 24 form sheets on two attached pads. None were used all blanks.Condition as shown in the photos. Paper has some age and use, rub and scuff, soiling marks, some paper discoloration. Pre-owned being sold "as is" as was found.
