This was the supplement order form in the Pressbook that movie theaters and drive-ins would order from in order to advertise the film in the newspaper at their venue and they were given multiple choices of artwork. This is an incredibly rare find. The Pressbook supplement measures 11x17 and is 15.5x22 inches in frame. Nick Castle, who played the Shape aka Michael Myers, autographed this rarity on the front and his autograph was authenticated by JSA and is serial # AI35464. You can look up the serial by searching JSA authentication look up online which will take you to their database. Nick Castle went on to direct "The Last Starfighter" as well.
