Includes the pop-gun and paper bang. I purchased this the day it was issued at my local K-mart. I left the prizes along and never touched them. I don't know where the order form is - I don't think one was included. Priced to sell as the gun alone brings over $50 routinely. Condition: VG+ LP (I played it several times). Cover is EX with original shrink (it does have a hit at bottom edge near right side (you can see in photo #3). Gun and paper bang are untouched. Sleeve is nice (I'll house the LP in a new Taguchi anti-static sleeve.
